Nurse Practitioner/Physician Assistant - Outpatient Medicine - Oswego
The Advance Practice Provider will deliver comprehensive patient care for the Oswego Division of Hematology/Oncology and the Department of Medicine in their outpatient practices, traveling to other oncology sites as needed. The Advance Practice Provider will work in collaboration with the Attending physician and will provide comprehensive health assessments and develop a plan of care for the patient population. Will also document in the medical record data in a consistent, legible, accurate, concise and timely manner. Will provide patient/family education and utilize prescriptive authority working in collaboration with the Attending.
An Advance Practice Provider is a member of the multidisciplinary healthcare team who demonstrates professionalism and leadership within the institution, who exemplifies the Upstate mission, vision and values and acts in accordance with Upstate policies and procedures.
Minimum Qualifications:

Position requires a NYS licensed/registered NP or PA required. Master's Degree as a Nurse Practitioner or as a Physician Assistant and current National Certification consistent to the clinical practice also required. NP requires National Certification through AANP or ANCC, and Master's Degree. American Nurses Association certification as a Psychiatric Nurse Practitioner required. CPR-BLS Certification required at the time of appointment. Prior experience in specialty area preferred. Monday-Friday Day Hours.
